Bestcare Solutions Pty Ltd is an approved NDIS provider based in Queensland. We provide support services across a range of NDIS registration groups, including community participation, daily living support, household tasks, life skills development, personal activities, transport assistance, life stage transition support, shared living support and group/centre-based activities.

We are currently seeking a dedicated, experienced and compassionate Community Worker to join our team and support participants to improve their independence, community participation and overall quality of life.

About the Role

The Community Worker will be responsible for supporting NDIS participants with their social inclusion, community access, personal development and service coordination needs. The role requires strong communication skills, accurate report writing, professional file note keeping, and the ability to work with families, carers, allied health professionals, support coordinators and other service providers.

Key Duties and Responsibilities

  1. Assessing participants’ individual needs, goals, abilities and support requirements.
  2. Assisting participants to access community activities, appointments, education, employment, recreation and social programs.
  3. Supporting participants to develop daily living skills, social skills, independence and confidence.
  4. Preparing progress notes, incident reports, file notes and participant support documentation.
  5. Liaising with families, carers, support coordinators, allied health professionals, government agencies and other service providers.
  6. Assisting in the implementation of participant support plans and behaviour support strategies.
  7. Supporting participants with life stage transitions, community participation and personal development goals.
  8. Monitoring participant progress and reporting changes in behaviour, health, safety risks or support needs to management.
  9. Providing transport assistance and community access support where required.
  10. Encouraging participants to build independence, maintain dignity, and make informed choices.
  11. Supporting participants with household tasks, daily routines and personal activities where required under their NDIS plan.
  12. Maintaining professional boundaries, confidentiality and accurate records in accordance with NDIS requirements.
  13. Working with participants who may have complex needs, including experience with restrictive practices and Module 2A-related supports where applicable.
  14. Participating in team meetings, case discussions, risk reviews and service improvement activities.
  15. Ensuring all support is delivered safely, respectfully and in line with NDIS Practice Standards and organisational policies.

Required Qualifications

  • At least a Diploma of Community Services or a closely related qualification.

Required Experience

  • Applicants must have at least two years of relevant experience in community services, disability support, NDIS support work, community participation, case support or a closely related role.

Required Licences, Checks and Documents

  • Current NDIS Worker Screening Check
  • Current Blue Card
  • Current First Aid Certificate
  • Current Australian driver licence
  • Reliable vehicle suitable for work-related travel
  • Current National Police Check or AFP Police Check
  • No suspension history on driver licence
  • Reference letter or statement of service from previous employer

Key Skills and Attributes

  • Strong understanding of NDIS participant support needs
  • Experience preparing file notes, progress reports and support documentation
  • Ability to assess participant needs and communicate concerns professionally
  • Strong verbal and written communication skills
  • Ability to work with allied health professionals, support coordinators and external agencies
  • Experience supporting participants with community access and participation
  • Understanding of safeguarding, dignity of risk, privacy and confidentiality
  • Experience working with participants with complex needs
  • Ability to follow behaviour support plans and restrictive practice requirements where applicable
  • Reliable, respectful and professional approach to participant support
  • Ability to work independently and as part of a team
  • Strong time management and problem-solving skills
