As our Senior Case Manager, you will build strong relationships, coordinate care planning, support foster care placements, and maintain quality documentation to ensure children, young people and carers receive the right support at the right time.
The program provides complex, intensive and general Home Based Care for children and young people who are unable to live with their own families. Most children and young people are placed in out of home care following child protection intervention. A small number are in care on a voluntary basis, voluntary and statutory placements can be of a short or long term nature.
As a Senior Case Manager, you will provide high‑quality statutory case management on behalf of the DFFH for children and young people living in foster care. You will also play a key role in supporting foster and kinship carers, contributing to placement stability, and strengthening practice across the program.
You are a compassionate and capable senior practitioner who can balance accountability with empathy. You thrive in a fast‑paced environment and are confident working with complexity and ambiguity.
