Berry Street Yooralla's Housing Services team sits within the Specialist Care and Transitions North West team within the wider Complex Care program. The team provides multidisciplinary, trauma-informed support to young people and families experiencing vulnerability, disengagement and housing instability.
The Case Manager, Housing Services will provide a case management response to clients with or without accompanying children who are experiencing homelessness or at risk of becoming homeless. Clients who access these supports are often in crisis and have multiple, complex needs making them vulnerable and as such the program aims to identify and address the issues that have led to their current situation. The program focuses on principles of social inclusion and social justice to ensure that those who have been overlooked by every other element of the system have a place to go.
We are looking for a passionate and values-driven practitioner who is committed to supporting people experiencing housing instability. You will bring a client-centred, strengths-based approach and thrive in collaborative, community-focused environments.
