Join our Adolescent Support Program and make a lasting difference in the lives of vulnerable young people aged 12–17 and their families. Based in Noble Park, this outreach-focused role works with young people who may be at risk of entering out-of-home care, helping them build stability, strengthen family connections and access the support they need to thrive.
As a Case Manager, you will provide intensive case management, assessment, advocacy and support to a caseload of young people. Working collaboratively with families, schools, Child Protection and community services, you will play a critical role in helping young people achieve positive outcomes while remaining connected to their family and community wherever possible. This is an opportunity to join a dedicated team committed to strengths-based, culturally safe and person-centred practice.
To be successful in this role, you will bring a passion for working with young people and families, alongside strong case management and relationship-building skills. You are adaptable, resilient and thrive in a dynamic outreach environment.
