About the Role

  • Work with consumers accessing NDIS support services who are living with a disability, mental illness and ageing within the community
  • Assist individuals, their families, and carers to navigate the health and social care systems, including NDIS
  • Support individuals in setting & achieving goals and outcomes that align with their needs and desires
  • Help to build the capacity of customers & their families to become more independent in managing their own care
  • Ensure customers' immediate and anticipated needs are met using a team approach

A bit about you

To have success in the role you will have a can-do attitude and the ability to hit the ground running. You will also have;

  • Empathy and passion to work with aged care, disability, and the mental health sectors
  • Ability to be a catalyst - facilitating change relating to a persons care
  • Eagerness to collaborate and join a strong group dynamic
  • Experience working in community-centred care systems
  • Excellent interpersonal skills + highly developed communication skills
  • Practical and problem-solving skills
  • Great tech skills and ability to navigate software
  • Previous experience in aged care, disability or support coordination will be highly regarded
  • Ability to work remotely in changing COVID-19 context
  • Social Work Graduates highly encouraged to apply
  • A valid NDIS Workers screening check and vaccination against COVID-19 (as per government mandate)
