Ballarat Community Health (BCH) provides a broad range of services to the community of Ballarat, with outreach services delivered in the Central Highlands region and beyond. In partnership with the community, we create opportunities and supportive environments which empower people to develop and maintain optimal health, as well as preventing and managing illness.

The Opportunity

The Multiple and Complex Needs Initiative (MaCNI) is a time-limited service response for people 16 years and older with multiple and complex needs.

Clients will present with complex issues including but not limited to mental illness, substance use, intellectual impairment, and acquired brain injury, and will benefit from a coordinated service response to meet their needs and goals.

MaCNI is funded by the Department of Health and Human Services, the Department of Justice and Community Safety, and the Director of Housing.

Focusing on a more effective and coordinated approach to support, MaCNI aims to:

  • Stabilise housing, health, social connection and safety issues
  • Pursue planned and consistent therapeutic goals for each individual
  • Provide a platform for long-term engagement in the service system.

You will have:

  • A relevant tertiary qualifications and/or experience in mental health, social services or allied health
  • Previous experience in care planning and coordination is preferred
  • Proven ability to provide excellent service to client, staff and general community members and the ability to show empathy and patience towards clients and staff
