As the End-of-Life Care Coordinator, you will be responsible for the development, coordination, and delivery of the new program. You will bring proficiency in service development, leadership and client care that fosters a culture of best practice, innovation, and continuous improvement.
Through this unique opportunity, you will also aim to ensure that individuals living with life limiting illness receive high quality care and support tailored to their specific needs.
