YOUR NEW ROLE

Occupational Therapists funded under the Home and Community Care Program for Younger People (HACCPYP) program provide services to clients under the age of 65 (under 50 for Aboriginal and Torres Strait Islander people), some of whom have a functional disability that risks a loss of independence.

The role also has a particular focus on advocacy and support for community members who are requiring assistance to access the NDIS. This will involve completing clinical assessments and functional capacity reports as indicated in order for clients to access the NDIS.

This position interacts closely with the multi-disciplinary team and other service providers, both internally and externally, in particular with local area coordinators (Brotherhood of St Laurence in the Northwestern metropolitan region).

YOU WILL BE RESPONSIBLE FOR:

• High-quality individualised assessment and management of clients using a strengths-based approach
• Assessments performed through home, community visiting and site-based avenues
• Completion of relevant documentation and report-writing for NDIS functional capacity assessments
• Attendance and participation in regular clinical support meetings & quality project planning
• Participation in delivery of group education sessions
• Participation in multidisciplinary care co-ordination, treatment planning and goal direct care planning for client care
• Attendance and participation in regular clinical support meetings & quality project planning
• Opportunity to be involved in OT student placement planning, supervision and feedback.

WHAT YOU NEED TO SUCCEED

• Current Occupational Therapist registration with AHPRA.
• Tertiary qualification in Occupational Therapy
• Minimum 2 years' experience working as an Occupational Therapist

Desirable:

• Experience working in community health settings and supporting clients from CALD and diverse backgrounds.
• Encourage experienced Grade 1's to apply to work in a dynamic and well-supported community health environment passionate about providing values-based healthcare to our clients
