About the role?

The Support Coordinator will report to the Coordinator- Client Services and is responsible for ensuring that flexible respite services are planned and implemented for eligible recipients under the CHSP guidelines. The incumbent will also play an important role in providing client support towards the transition to person directed supports, which may include supporting the care recipients and their network to explore alternative options other than traditional support models. They will ensure that suitable support staff are engaged while reviewing all flexible services and making adjustments as required.

The incumbent will also ensure that reporting, documentation and records requirements are completed as per annecto policies/procedures and Government legislation. The Support coordinator will be required to participate and contribute to team projects and undertake other relevant duties as required.

Requirements:

  • Demonstrated commitment to the Purpose and Principles of annecto - the people network;
  • Knowledge and understanding of the National Disability Insurance Scheme, the disability and Aged care sector;
  • Understanding and commitment to person centred approaches;
  • Demonstrated ability to work efficiently with culturally diverse groups;
  • Demonstrated ability to work autonomously with a high degree of initiative;
  • Excellent communication skills, both written and verbal;
  • Previous experience in support coordination in a community setting;
  • Well-developed time management skills, and the ability to prioritise;
  • Current unrestricted Victorian Drivers licence.

What would be great to have?

  • Experience working in a similar capacity in a community based not-for-profit organisation.
  • Knowledge of client data bases e.g. Carelink
  • Training and experience in Person Centred Planning
  • An ability to speak a community language
  • Knowledge of DEX reporting
  • Ability to speak a community language/s
