About the Opportunity

Working closely with the Manager Youth & Family Services, you will coordinate the administrative and reporting requirements of the Housing Options Pathways program (HOPP) while delivering high-quality case management support to vulnerable individuals and families.

Key responsibilities include:

• Delivering strengths-based, client-centered case management.
• Conducting intake, assessment, case planning and supported referrals.
• Assisting clients to develop tenancy management and life skills.
• Advocating for clients and supporting housing applications.
• Building strong relationships with community organisations, housing providers and other stakeholders.
• Maintaining accurate client records, data collection and reporting requirements.
• Facilitating the weekly Food Matters Breakfast Program for people experiencing homelessness.
• Providing outreach support to people sleeping rough and assisting them to access accommodation and support services

About you

You are compassionate, resilient and able to build trusted relationships with people facing complex challenges.

To be successful in this role, you will bring:

• Certificate III in Community Services (or equivalent), and/or relevant experience in community services, case management or family support.
• Experience developing and implementing client support plans.
• A strong understanding of issues impacting people at risk of homelessness and tenancy failure.
• Excellent communication, administration and report-writing skills.
• Experience working collaboratively with government, non-government and community stakeholders.
• Strong organisational skills and the ability to manage competing priorities.
• A commitment to culturally safe and respectful practice when working with Aboriginal and Torres Strait Islander peoples and culturally diverse communities.
