Anglicare Victoria Youth Services are a provider of the Better Futures Leaving Care and Post Care service in the Eastern Region. The Better Futures program is an innovative program aimed at providing secondary consultation for young people aged 15 years and 9 months to 17 years and 5 months and intensive case management to young people aged 17 years and 6 months to 21 years of age, who are currently or have previously resided in Out Of Home Care and require additional support in education, employment, health and well-being, connections and housing and living skills, using an Advantaged Thinking Model.
The role provides a flexible outreach and case work approach that will assist young people to successfully engage, or re-engage in education, employment, health and well-being, connections and housing and living skills, using an Advantaged Thinking Model. The Case Manager will conduct assessments of young people and identify their needs, to create and support the achievement of goals in education, employment, health and well-being, connections and housing and living skills. You will work collaboratively with key stakeholders to strengthen service delivery outcomes for young people at risk, for example by attending care team meetings, network meetings and service planning meetings
Please Note: It is mandatory for the successful applicant to hold a current Working with Children Check and be prepared to undergo a National Criminal History Check prior to commencement.
