About the Role:

The Intake and Assessment Officer is the first point of contact for prospective clients seeking support. In this role, you will conduct comprehensive intake and assessment conversations to understand each client's needs, circumstances, preferences, and goals. You'll help connect clients with the most appropriate Anglicare Southern Queensland services or refer them to external community support services where needed.

We are looking for an Intake and Assessment Officer to join our team in a permanent, full-time position 38 hrs/week based in Strathpine.

Responsibilities:

• Conduct client intake and assessment interviews via phone and online to determine eligibility and support needs.
• Connect clients with appropriate Anglicare services or provide warm referrals to external support agencies.
• Undertake client risk assessments and develop safety plans where required.
• Advocate for client choice, rights, and person-centred service delivery.
• Build and maintain strong relationships with government and community service providers.
• Maintain accurate client records, case notes, and service documentation.
• Collaborate effectively with internal teams and external stakeholders to achieve positive client outcomes.
• Participate in team meetings, supervision, training, and professional development activities.
• Contribute to continuous improvement initiatives, service development, and quality outcomes.
• Ensure compliance with organisational policies, legislative requirements, and professional standards.

Qualifications, Credentials and Other Requirements

• Current National Police Check
• Current Blue Card (Working with Children)
• Degree in human services, social work or psychology or similar.
