You will be responsible for the delivery of specialist and intensive assistance to support older people to access aged care services and other relevant supports in their local community. You will provide assertive outreach services to connect and engage with people who meet the eligibility requirements for the Care Finder program, working with internal and external contacts to ensure access to services and maintenance of support required to live independently in the community.
You will hold relevant qualifications and experience in homelessness or aged care services.
You will uphold the values of Anglicare NT and provide quality services within the scope of the position and associated delegations.

Please note: It is a requirement that prior to commencing in this role, you must possess a Working with Children (Ochre Card), a National Criminal History Check (within 3 months) and a current Northern Territory driver’s licence.
