About the opportunity

Due to our organisational growth and increase in our client numbers serviced through our HCP Program we are seeking to appoint two new Team Leaders to support our Care Advisor team and the services provided to our valued clients.

As our Team Leader it's vital you've had at least 2 years experience working within the HCP Program in order to lead our teams successfully. You will provide leadership and support to the Care Advisor team to ensure the provision of services and care are delivered to the highest quality with reference to the Aged Care Quality Standards. You will also work in partnership with clients and their representatives to facilitate the consumer directed support and services as required to meet the client's goals and preferences in terms of support to live safely and contentedly at home.

About you

You are an Aged Care Services professional, and an established leader. You've looked after your own team for 2+ years just as well as you have overseen those in care. You're a trusted advisor, a tireless advocate, with empathy in spades. But you also have that crucial big-picture outlook. Your knowledge of Home Care packages and guidelines will be vital to your success. You are looking for that next opportunity that will support your plans to become a senior member of the management team.

Reporting to the Manager of Home Care Package you will thrive in this role as you:

• Provide leadership to a case load of Care Advisors
• Provide monthly supervisions and annual appraisals
• Oversee complaints handling while strengthening client to care advisor relationships
• Manage and assess performance of your team while championing quality of care improvements
• Drive the professional development of the team, facilitating career progression
• Identify and implement service innovation and improvements to maximise both client and operational outcomes under the HCP program
• Apply your cultural competency skills across a proud multicultural organisation

Drawing upon your extensive knowledge and experience in Home Care Packages, paired with your tertiary qualifications and Aged Care / Community services background will be essential to succeeding in this role. Your enthusiasm for continuous improvement and natural ability to inspire those around you to deliver high standards of work, is what will see you easily step into this role, and position yourself for our future succession plans!
