We have an exciting opportunity for a Clinical Coordinator to join our HealthPathways team. This role works closely with all HealthPathways team members to progress the HealthPathways program.

HealthPathways is an online portal that provides general practitioners and other health professionals with easy access to comprehensive, evidence-based assessment, management and localised referral resources for specific health conditions.

The HealthPathways localisation process involves working collaboratively and in partnership with local clinicians and service providers relevant to specific health conditions. Using base HealthPathways, standardised, easy to follow guidelines for patient care are localised for the assessment, management and patient access to local services. This information is published on the HealthPathways SA online portal as a central source for health professionals.

The Clinical Coordinator will:

  • Coordinate the HealthPathways content and feedback process, including:
    • Gathering non-clinical information from stakeholders
    • Coordinating a range of communications to support the GP Clinical Editors, HealthPathways team and HealthPathways process
    • Collating and coordinating the required information to support localisation of HealthPathways.
  • Monitor feedback as recorded in the HealthPathways administrative tool or other mechanisms, and action as required in a timely manner, aligning with other stakeholders and HealthPathways Team members.
  • Collect and maintain local service provider information including outpatient clinic information, community based services, private sector information and any other relevant information as identified by the HealthPathways Team.
  • Maintain project plans and milestones, with a focus on key outcomes.
  • Work in partnership and collaboration with individuals and organisations to maintain key stakeholder relationships including:
    • GP Clinical Editors, Subject Matter Experts and Clinical Champions
    • SA Health
    • Primary Health Networks
    • Streamliners
  • Coordinate Clinical Working Groups as required including administrative activities and reporting.
  • In collaboration with other members of the HealthPathways Team, plan for live pathway release in conjunction with relevant stakeholders and support implementation activities as required.

You must be passionate about and committed to driving positive change throughout primary care and improving health outcomes.

We expect the successful applicant will have a tertiary qualification in a health related area and/or extensive experience in health related project management and delivery.

You will be able to identify problems, challenges and barriers, and recognise opportunities, and come up with potential solutions. You enjoy a challenge and finding solutions.

You may have had experience in previous roles in the health sector such as: Project Officer, Project Support Officer, Team Leader, Coordinator Health Officer, Primary Health Care Officer, Allied Health Professional.
