Purpose of the Disability Support Worker role

As a Senior Support Worker at Achieve Australia, your primary goal is to provide person-centred quality support services to our client based at Nepean Hospital. You will have solid experience supporting disability clients with complex health needs and challenging behaviours. You will also be able to work on a 24/7 roster with active nights.

Role Responsibilities:

As a Senior Support Worker, you will work at Nepean Hospital and support this client to build independence and communication. Your duties will include:

  • Promote the delivery of supports in accordance with Individual Support Plans to assist the client to remain safe and healthy.
  • Support team members to implement Individual Support Plan and to gather and analyse information to identify issues and determine if changes are required to meet clients individual needs.
  • Record and report identified change requirements
  • Complete mandatory documentation and any supporting documentation accurately and within given timeframes
  • Provide and promote timely reporting on incidents, risks and complaints
  • Monitor the implementation of behaviour support plans
  • Participate in staff briefings and undertake training to ensure compliance with the organisation’s policies, processes and procedures, to meet the NDIS Practice Standards, Quality Indicators and all regulatory and legislative requirements.

To be successful in this role, you will need:

  • Demonstrated ability to take initiative and work independently in providing support to individuals with complex needs, including a track record of proactive problem-solving.
  • Proven resilience in high-pressure situations, particularly when dealing with physical and verbal aggression, showcasing an ability to maintain composure and provide effective support during challenging moments.
  • Substantial experience working with individuals with high support needs, complex health requirements, and restrictive practices, highlighting the ability to navigate and implement comprehensive support strategies.
  • Strong communication skills, particularly in adapting communication methods for individuals with impaired communication, ensuring effective understanding and responsiveness to the unique needs of the person being supported.
  • Consistent and reliable work history, with a proven ability to meet responsibilities and commitments, including adherence to schedules and protocols essential for providing continuous care.
