Our Client, an established not-for-profit organisation is currently looking for a motivated and passionate Case Worker to join an established Homelessness case work team.

This role will involve:

• Conduct assessments to identify client needs, assess risk understand barriers
• Work with clients to identify and prioritise housing, health and support goals
• Work on person centred action plans and interventions in partnership with clients, colleagues and other agencies
• Provide a professional, supportive and effective service
• Follow policies & procedures including WHS
• Continue to develop links with other relevant programs, government agencies and non government group

The requirements for this role include:

• A qualification in human services,
• 2 years experience working with vulnerable people in Mental Health, AOD, Domestic Violence or Homelessness advantageous
• Knowledge of local services, ability to continuously build relationships with key stakeholders
• Good organisation and time management skills
• Strong written and verbal skills
