Our Client, an established not-for-profit organisation is currently looking for a motivated and passionate Case Worker to join an established Homelessness case work team.
This role will involve:
- Conduct assessments to identify client needs, assess risk understand barriers
- Work with clients to identify and prioritise housing, health and support goals
- Work on person centred action plans and interventions in partnership with clients, colleagues and other agencies
- Provide a professional, supportive and effective service
- Follow policies & procedures including WHS
- Continue to develop links with other relevant programs, government agencies and non government group

The requirements for this role include:
- A qualification in human services,
- 2 years experience working with vulnerable people in Mental Health, AOD, Domestic Violence or Homelessness advantageous
- Knowledge of local services, ability to continuously build relationships with key stakeholders
- Good organisation and time management skills
- Strong written and verbal skills
