My client is looking for a Permanency Case Manager in the ACT. As a Permanency Case Manager, you will be responsible for the case management of children, young people and families in the ACT under the Child Protection Intervention and OOHC.

This role will involve:

• Supporting foster carers and undertaking restoration, adoptions, guardianship and long term OOHC placements.
• Deliver therapeutic case management underpinned by the Functional Family Therapy Model
• Work toward family preservation and/or restoration with families if this can not be achieved working toward adoption, guardianship or long-term foster care
• Undertake complex case management and detailed reporting requirements
• Conduct investigations into reportable conduct allegations regarding carers
• Undertake and complete all necessary data collections, case notes and reports within specified timeframes

The requirements for this role include:

• Diploma or Degree in Community Services, Social Work or a related discipline
• Experience working within a trauma-informed practice
• Experience working to support children, young people and parents or carers
• Knowledge of therapeutic case management, assessment and development of purposeful case plans
• Valid Working with Children Check or Working With Vulnerable People Check as required
