The Kinship Care Case Manager is responsible for comprehensive case management and ongoing support for carers and children in statutory kinship care placements. This includes visits, identifying support needs, and enhancing cultural competence, especially for non-Aboriginal carers.
The role also involves completing all case management tasks, court reports, and assessments, utilising the Looking After Children Framework to ensure stability for Aboriginal children. Additionally, the position may require conducting Part B Kinship Assessments for the First Supports Program.
Provide ongoing support to carers and children in statutory kinship care placements, including fortnightly visits. Identify and address the training and support needs of kinship carers, with a focus on enhancing cultural competence for non-Aboriginal carers and ensuring Cultural Support Plans for all children. Facilitate family support and contact between children and their families. Use the Looking After Children framework to assess and meet children’s needs, complete case management tasks, and report to DFFH.
Update client data on CRISSP and CRIS systems, and complete necessary reports and assessments. Manage cases across contracted and First Supports programs, and participate in supervision, training, meetings, and quality improvement activities. Handle cases involving foster carers or targeted care packages as needed.
