About the role

VincentCare Victoria is seeking an, energetic, and committed Registered Nurse to undertake clinical assessments for HCP clients, provide oversight for nursing and clinical services, and provide consultation and guidance to both the Case Management and Support Worker teams. This is performed in accordance within the scope of practice of a RN as determined by the Australian Health Practitioner Agency (AHPRA) and the Nursing and Midwifery Board of Australia.

Other key functions of the role include:

  • Conducting clinical assessments and review, liaising with mainstream and specialist health providers, and providing secondary consultation with other program staff.
  • Coordinating the development, implementation and ongoing review of clinical assessment and care planning.
  • Reviewing and responding promptly, effectively and appropriately to changes in client's health status.
  • Ensuring client care is delivered in accordance with the Aged Care Quality Standards and Organisational values, standards, policies and procedures and to the concept of continuous quality improvement.

What We Are Looking For:

  • Current registration with Australian Health Practitioners Regulation Agency (AHPRA) as Registered Nurse (Division 1) with no restrictions on registration to practice.
  • Aged Care experience - preferably community based.
  • Able to travel to Both Gippsland and Shepparton regions to conduct clinical assessments for clients in these regions.
  • Understanding of accreditation process and the requirements of the Aged Care Quality Standards as they apply to in a Community setting.
  • Be able to work autonomously and collaboratively within a team setting.
  • Understanding of, and ability to undertake clinical assessments and develop or inform care plans.
  • Ability to take necessary initiatives and make appropriate judgments regarding clinical care.
  • Excellent customer service skills.
  • Empathy, patience, and a strong sense social justice.
