About the role

VincentCare Victoria is seeking an, energetic, and committed person to take on the position of Case Manager to deliver a high-quality care and support to Home Care Package clients in the Inner-Melbourne region. We are seeking an appropriately-qualified and experienced, enthusiastic community Aged Care professional to provide independent, ongoing advice and guidance to our clients.

This role will involve creating partisanship's with the client to design and plan services & support systems that ultimately meet the needs and aspirations of our clients.

You will provide case management advice and support that is respectful of, and responsive to the preferences, needs and values of individuals. You work collaboratively with Aged Care - Home Care Package clients, their carers and representatives to assess needs, provide consumer choice and develop sustainable, flexible, personalised and culturally appropriate care plans to meet their personalised goals.

Within this role you will:

  • Review intake information, engage new clients, establish the home care agreement, person-centred care plan, budget allocations and connect clients with local services and supports to meet their goals.
  • Assist clients in the understanding of their package spend and the choices available to them
  • Network with team members, stakeholders, and partnered service providers on behalf of our clients
  • Conduct annual reviews for all clients, revise care plans, goals and services to maximise budget utilisation
  • Practice within applicable legislation while recognising and respecting the client’s right
  • Actively promote and refer clients to VincentCare services
  • Implement best practice quality models of assessment, care plan development, implementation and evaluation

You are friendly, compassionate, customer service focused & qualified professional with experience in providing person-centred, individualised care and support to clients. You have experience in maintaining constructive, empathetic, supportive and respectful working relationships particularly for an integrated service response. You are passionate about caring for others.

What We Are Looking For

To be successful, you have:

  • Tertiary qualifications in allied health, social science, health/welfare discipline, or a related field
  • Demonstrated experience with person-centred care planning processes, including assessment and establishing client goals
  • Demonstrable experience in case management of government funded packages, community resources, and relevant legislation in the provision of aged care services
  • Experience managing client budgets, being responsive and accountable for time sensitive tasks
  • A proven ability to take a holistic approach to client needs and service delivery
  • An understanding of the issues experienced by aged care clients and their carers in the home and the differing, social, spiritual and cultural needs of clients
  • Exceptional interpersonal and communication skills to demonstrate empathy and understanding to older clients and their families
  • Excellent customer service skills
  • A positive and proactive approach to working collaboratively within a team and the broader community
  • An ability to take initiative, prioritise, delegate and organise person-centred activities
  • Empathy, patience, and a strong sense social justice.
