Part time (25 hours per week), ongoing
Based in Tiwi Islands
As a PHaMs (Personal Helpers and Mentors) Community Case Worker you will build long term relationships with diverse participants and work to encourage their social integration with the community. You will provide emotional and social support using a strengths-based and recovery focused approach. Carrying out assessments, reviews, referrals and case planning will be required.
You will have excellent listening and communicative skills as well as an ability to plan and organise activities. Your ability to build strong interpersonal relationships and collaborate with relevant service providers will be highly regarded. You will also hold a First Aid Certificate and a Certificate IV in Community Services and/or Mental Health will be required or a willingness to obtain one.
