Our Specialist Support Coordinators are NDIS experts in offering Level 2 and Level 3 Support Coordination for people with a NDIS plan. We connect customers to their communities and find the best outcome for their needs.

About You

We’re looking for a collaborative, resourceful and customer-oriented team player with exceptional customer service skills.

To be successful in this role, you’ll need to have:

  • Two plus years of experience in Support Coordination/Case Management role in the NDIS, disability or aged care.
  • Qualifications in Social Work; Degree/Diploma in Disability or Community Development; Allied Health, Psychology, or Case Management.
  • Working with Children Check.
  • Current Victorian Driver’s license and well-maintained vehicle that meets current safety standards with comprehensive car insurance.
  • NDIS Worker Screening Check clearance.
  • Nationally recognised Police Check.
  • Eligibility to work in Australia.

It is an employment requirement that you’ll complete the NDIS Quality and Safeguards Commission online Worker Orientation training module called ‘Quality, Safety and You’ prior to your commencement (if you don’t already have evidence of this certification).

To be successful you will also:

  • Be a quick learner with great communication skills.
  • Possess good computer skills (intermediate knowledge of Excel).
  • Be able to set priorities and manage time effectively.
  • Be able to work remotely and independently while still contributing to a team environment.

About the role

As a Specialist Support Coordinator, you will provide varying levels of coordination and transition supports for children, young people and adults, Considering their unique circumstance and level of complexity.

Your primary focus will be to support people living with complex needs to thrive.

Your responsibilities will include:

  • Coordinating a range of supports to suit customer needs across multiple providers
  • Coordinating a customer’s NDIS plan to enable greater independence across all domains of their life
  • Building a customer’s capacity (and that of their networks) with the aim of identifying the mechanisms to increase independent management of issues and supports to meet their NDIS goals.
  • Contributing to the learning of complex case work of other Support Coordinators across OnSide and where appropriate with external providers.

This is a permanent full-time position. Hybrid work model with flexibility to work from home. Our offices are in located in Notting Hill, however frequent travel will be required across various metropolitan and regional locations to support our customers.
