Are you someone who loves bringing order to complexity, building genuine relationships, and making a real difference in people’s lives? My Place is looking for a Co-ordinator to join our Lower Great Southern team, supporting people with disability to live the life they choose.
This is a hybrid role based in Albany, offering flexibility, variety, and the chance to work closely with consumers, families, support workers and a collaborative Perth based team. No two days look the same, and that’s part of the appeal.
30–38 hours per week (spread over 5 days – negotiable for the right person)
My Place is an established for-purpose provider of individualised and flexible supports for people with disability and their families. We empower around 400 individuals to live independently in their own homes or with their families, promoting their growth as valued and contributing members of the community. Importantly, we do not operate group homes, day centres, or other congregate care services.
We offer our organisational staff a 9-day fortnight, flexibility for family commitments, a phone and laptop, 5 weeks of annual leave, and 13 weeks of long service leave after 7 years, along with numerous health and wellbeing perks.
As a Co-ordinator, you’ll be responsible for the individualised co-ordination of supports for people with disability, ensuring their NDIS plans are understood, implemented and working well in practice.
You’ll work across a mix of responsibilities, including:
Consumer Co-ordination & Support
Provide person-centred co-ordination of supports, assisting consumers and significant others to implement, monitor and review NDIS plans and achieve their goals.
Your Workplace & Staffing Support
Support Your Workplace arrangements by assisting with recruitment, induction and supervision of Personal Assistants to ensure continuity and quality of supports.
Collaboration, Coverage & Supervision
Work collaboratively with internal teams while providing leave coverage and supporting consumers and families to meet employment and engagement obligations.
Administration & Systems
Maintain accurate records, documentation, budgets and funding claims, and liaise with relevant funding bodies and internal teams as required.
You’re organised, approachable and confident juggling multiple priorities. You care deeply about person-centred practice and are comfortable working both independently and as part of a team.
