about the role

Care Coordinators will provide Social Support and respite for eligible participants over 65 years of age under Community Home Support Program funding. Key aspects of the role include assessment, care planning, bi-annual reviews and face to face home visits with participants to identify individual goals and strategies to help improve quality of life. These positions are part of the Aged, Chronic Care and Allied Health Stream of Belong Blue Mountains.

All Care Coordinators will work closely with volunteers who are integral in providing social support to both our participants and their carers. All eligible participants are referred via the My Aged Care Portal.

These positions will be covered by the Social Community and Home Care Award (SCHADS) Level 3, with above Award conditions available.

The position will commence in early 2022 and all applicants must have proof of full COVID 19 vaccination status.
