The Diabetes Educator works in partnership with clients and their families/carers to provide health education, therapeutic management and support services as required for diabetes prevention and/or management within a chronic condition self-management framework. The Diabetes Educators work within a social model of health and with the broader team to address identified problems with clients.
The Diabetes Educator will also be working in our Diabetes Wellbeing Hub and is a key member of the multidisciplinary team working alongside Allied Health, an Endocrinologist and General Practitioners, as well as a Wellbeing Coordinator to provide integrated and coordinated person centred care to improve outcomes for people living with diabetes.
We are seeking a Credentialed Diabetes Educator, or working towards status through the ADEA credentials program. You will have excellent communication, negotiation, organisational and time management skills. You will also have a strong understanding and commitment to the principles of community health and have the ability to work autonomously as well as part of a multidisciplinary team. Experience with working with people from a culturally and linguistically diverse or refugee background is desirable.
