Established in 1982, Alzheimer’s WA provides care, support, awareness, and information to assist people living with dementia, as well as their families and carers.
Our services include advocacy, education, partnerships, training, consultancy, and holistic person-centred care programs, based on a philosophy of understanding the lived experiences of those living with dementia. We provide CHSP and HCP home care packages, and day and overnight respite in our three homes based in Perth, Mandurah, and Albany.

Alzheimer’s WA is seeking an experienced HCP Coordinator with the enthusiasm, care, and compassion to support people living with dementia based out of Mandurah.
This position coordinates the individualised supports and services for our clients from onboarding, through to review, to enable the client’s needs and goals to be met, throughout their dementia journey.
