The Australian Vietnamese Women’s Association Inc. is a growing public benevolent institution servicing Vietnamese and other culturally and linguistically diverse clients. Our income comes mainly from providing government-funded services for all ages and genders. Our main services include aged care, training, playgroups, justice and other services.
We are seeking to employ a skilled Care Manager for Home Care Packages Program on a full-time basis.
As a part of the role you will be required to undertake the following duties:
- conducting individual and family case interviews to identify clients’ goals, conducting client assessments including planning, monitoring, and regular reviews using a person centred, wellness and reablement approach and provide options to assist clients meet their needs within available resources
- acting as a facilitator between clients in need and community services, locating and arranging customised services required by the carer and consumer as per the agreed care plan liaising with internal and/ or external stakeholders as needed
- promote clients' access to resources, community supports, and services based on their care needs
- cooperating with community organisations, social agencies and voluntary groups to enhance clients' access to services based on their needs
- assisting clients to understand and resolve problems by providing information and referring them to relevant agencies and services
- monitoring the progress of clients by maintaining contact, evaluating individual goals and service outcomes
- compiling case records and reports, ensuring appropriate documentation, maintaining up-to-date consumer’s records
- Acting as an advocate on behalf of consumers for the rights, decisions, strengths, and needs of clients
The ideal candidate will possess the following skills and experience:
- Demonstrated experience and expertise in working with Vietnamese aged people and their families including counselling, assessment, and case management (at least 3 years of experience is preferred)
- Understand and sensitive to the needs of the Vietnamese aged people in the Australian context
- Administrative work skills such as staff rostering, case notes writing are essential
- Demonstrated knowledge of the aging process and aged care system, eg. My Aged Care
- Highly developed interpersonal communication skills, and the ability to liaise effectively with care staff, organisations and persons from diverse backgrounds and cultures
- Demonstrated knowledge of available interventions, support services and the appropriate referral networks
- Demonstrated ability to achieve positive outcomes and manage time to respond to the consumers’ changing needs and their care plans
- Demonstrated supervision and staff motivation abilities
- Ability to speak and write English and Vietnamese fluently
- Holding a current Victorian driver’s license
The successful applicant will be rewarded with salary of between $69,160 - $79,040 plus superannuation, Salary packaging is available.
