About the role

Positively influence and contribute to a team that focuses on meeting the customer’s goals using evidence informed practice. Reporting to the NDIS Transition Manager, you will have delegated authority to make day to day decisions to ensure the operational requirements of the service are met.

Duties include:

  • Assist staff to problem solve, develop creative solutions and remain motivated in meeting customer’s goals
  • Build an understanding and awareness of risks that are particular to staff working solo in the community
  • Plan, deliver, evaluate and review group work programs that will assist customers to build their skills and focus on their recovery
  • Make linkages and build relationships to maintain or create a range of local supports for customers

Skills & experience

  • Tertiary qualifications (minimum certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health related field as designated by Mind
  • Previous experience supporting people with mental health issues and complex needs i.e. alcohol and other drug issues, dual disability
  • Strong leadership and management experience
  • Demonstrated ability with case management work or care/support coordination services in a community-based environment
