Your new company

This community based not for profit organisation help connect individuals and communities, with aims of creating an inclusive society. They strive to provide excellent service delivery and the facilitation and strengthening of social supports, particularly through the participation in sector related policy development and review.

Your new role

In your new position as a case manager you will implement care package plans and monitor them through consulting with the client and their carers, as well as service providers and other significant parties. You will use person cantered and community based approaches and support the individuals to remain living in the community. In this program you will focus on engaging with the local community and supporting them, aiming to slow "functional decline" in older Australian people, with a focus on Aboriginal and Torres Strait Islander people.

This is a 24 month fixed term contract, working full time.

What you'll need to succeed

  • Social Work Tertiary Qualification or equivalent
  • Experience in assessment and case management within a community setting
  • Current drivers licence
  • Demonstrated ability to work autonomously
  • Demonstrated ability to work efficiently with culturally diverse groups
