Alzheimer's Queensland is a leading home and community care provider now expanding its in-home clinical care operations in the Brisbane South and Logan River Valley region.

We are currently seeking a Registered Nurse - Home Care Case Manager with a sound understanding of the principles of My Aged Care, Aged Care Reforms and the Home Care program to coordinate the clinical delivery of home care services.

With a diverse caseload you will have a pivotal role across the service delivery of aged and disability care, home care packages, as well as commonwealth home support program, transitional care, PRN, post-operative care and palliative care.

This role is customer focused and involves consulting with families, preparing clinical home care assessments and drafting care plans to best meet client requirements whilst prioritising your caseload.

In addition to care co-ordination duties this position will help develop relationships with hospitals, clinics and care facilities with a view of delivering care services to meet client needs.


  • AHPRA - Div 1. Nurse Registration
  • Current Opens Driver's Licence
  • Current NDIS Yellow Card or willingness to obtain
  • First Aid & CPR Certificate
  • Police Check, or willingness to obtain

Skills & experience:

Our ideal candidate joining the home care team will ideally have case management experience as a Registered Nurse in community aged and/or disability care with the ability to coordinate clinical home care services.

You will have existing relationships with care facilities, hospitals, and GP clinics to support business development opportunities. This person will have the ability to connect with others naturally and effortlessly and the confidence to market AQ services to potential clients and referral sources.

Additional talents to succeed in this role is your highly developed organisational, administrative and financial management skills with proven ability to determine priorities, coordinate activities and meet deadlines.

Excellent communication and interpersonal skills is a must, both written and oral as well as strong computer literacy with Microsoft and data base applications such as iCare.
