The Family Practitioner will be responsible for providing high quality, culturally appropriate, strengths-based, trauma informed intensive case management and flexible support with a focus on enhancing the lives of children, young people and families in Ngukurr. This will involve a mix of therapeutic, in-home practical and case management support to families facing challenges that may be complex, such as family violence, trauma, grief and loss, interaction with the child protection or justice systems or where children are at risk of harm.

The role will work directly alongside the Aboriginal Family Practitioner - working with families, in partnership with other local staff and social programs to develop and implement a Family Support Plan, which builds on strengths and knowledge of caregivers, engages with support networks, strengthens connections to family and community, encourages positive decision making and ability to self-manage and promotes safety and wellbeing.

Our ideal candidate will have:

  • The ability to provide professional child centred, family focused and friendly case management services and practical assistance in line with YMDAC policies, procedures, standards, contracts, work plans and legislation.
  • To build strong, trusting, and respectful relationships with families and young people to assist them to develop collaborative support plans and achieve set goals.
  • Ensure work with families and case management services are culturally safe, strengths-based, solution focused, and trauma informed with a focus on respectful relationships and practical outcomes in line with agreed case work plans and any group work programs.
  • Provide assertive outreach support to families, working where they feel safe.
  • Assist with the establishment and running of groups/events providing cultural activities that strengthen identity and wellbeing and increasing parenting skills and capacity.
  • Knowledge of the child protection system and domestic and family violence supports & other community service systems.
  • Experience in the application of case management frameworks, child and youth focused family friendly approaches and community capacity building strategies.
  • Experience in assertive outreach strategies and ability to effectively engage with families with challenging and/or trauma related experiences associated with child abuse, neglect and/or family breakdown and family violence.
  • Preferred Tertiary qualification in Social Work or Human Services / Social Science related fields. YMDAC will consider applicants who can demonstrate other appropriate qualifications for the role.
  • Northern Territory Working with Children Clearance (Ochre Card).
  • National Police Criminal History Report with acceptable outcome.
  • Northern Territory Driver's Licence.
  • First Aid Certificate (or willingness to obtain within agreed timeframe).
