Western NSW Primary Health Network (WNSW PHN) is currently seeking a motivated individual who has a passion for primary healthcare and community wellbeing.
WNSW PHN funds a range of health services and programs to improve access to care and better health for our communities.
The Project Officer - Older Persons & Palliative Care position supports the strategic objectives of WNSW PHN to develop A Healthier System, by coordinating activities to develop A Healthier System related to older people’s health, aged care and palliative care.
In response to the Royal Commission into Aged Care Quality and Safety and the impact of COVID-19 on older people’s health, the Primary Health Network seeks to increase activities focused on better access to health care for older persons, the aged care sector and people accessing palliative care at home.
Connecting Care through Digital Health in the aged and palliative health care sectors. The role will build on existing relationships and alliancing strategies and where appropriate build new connections and strategies to deliver the new programs to the target population.
