This is an exciting new role which will provide Weekend Support to clients of the Street to Home (S2H) Program, as part of the recently DHHS funded Modular Unit Program in Bacchus Marsh.
The position enhances the ability of individuals to be able to live independently, develop community engagement and access other services as necessary.

Applicants for this role will have a thorough practical working knowledge and understanding of person-centered approaches to working with clients who have a history of sleeping rough.
Clients will be housed in the Modular modular Unit Program. You will be highly organised and possess a comprehensive knowledge of supports and services available across health and community services.
You approach your work with a strong commitment to working proactively and are passionate about working with people who may have significant social and mental health issues.
