  • Permanent part-time position (7 days per fortnight until 30 June 2024 – then 5 days per fortnight ongoing)

As part of our Family Preservation and Reunification Response Program, the Finding Solutions Plus Program provides case management support to young people aged 10 to 15 years who are at imminent risk of being placed in out of home care for the first time. The program also supports young people to return home to their parents care where possible.

As a Case Worker you will:

  • Provide outreach assessment, mediation and a range of supports to young people
  • Support the development of therapeutic action plans
  • Use a strengths based approach to empower young people and their families to self-determine plans that promote the young person’s safety, development and permanency of placement
  • Maintain comprehensive case notes and prepare high level reports

About you

To be successful in your application you will meet the person specification in the position description, including having a tertiary qualification in Social Work, Psychology or a related discipline.

You will also have:

  • Sound understanding of the Best Interests Framework and Children, Youth and Families Act 2005
  • Extensive professional experience in the field of child, youth and family services, including experience working with or for Child Protection
  • Proven ability to engage young people and their families
  • Demonstrated ability to plan and implement appropriate interventions which strengthen family decision-making and relationships
  • Well-developed written and verbal communication skills
