We have an exciting opportunity for a passionate Care Manager to join our Support For Carers Support Program (SCP). The program provides tailored flexible support to carers who provide unpaid care and support to a family member or friend, who has a disability, mental illness, chronic condition or an older person with care needs.
As a Care Manager, you will support the health and wellbeing of carers by providing person-centred care and support. The Care Manager will provide one-off or short-term support to carers such as providing information, undertaking assessments and assisting with goal directed care plans. The role will also organise and facilitate social and recreational activities and events for carers.
