Wesley Mission has a long established history in providing Out of Home Care across NSW. This position offers a challenging and rewarding opportunity to work with young people aged 16 to 18 years within our Semi-Independent Living Program and Alternative Care Arrangements (ACA).
Wesley Mission is seeking 2 dynamic Case Managers to provide independent living services to participants in the Wesley Mission Alternative Care Services. Our Alternative Care Case Managers will be responsible for supporting youth in building skills necessary to function independently; facilitating the implementation of services as needed, responding to crises, and assisting youth in accessing educational, training, and community resources. Developing rapport and using a strength-based approach with the youth is essential.
Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010. The agency provides regular supervision and support for all staff and opportunities for professional development.
