About this role:

We recently successfully won a grant submission through NDIA to deliver a new program called the Exceptionally Complex Support Needs Program (ECSN). This program aims to improve sector capacity to provide support coordination and mainstream services to NDIS participants with exceptionally complex support needs as well as providing crisis management support directly to these participants.

We have been commissioned by NDIA to deliver the three functions of this new program across Victoria:

• Sector and Community Development
• Subject Matter Expertise
• Crisis Referral Service (including afterhours response)

This program and the contracts will run from November 2019 through to June 2021.

This position will work close together, whilst supporting delivery of all three ECSN functions (listed above). This position will support the wider team with the following program objectives:

• Support the growth and development of organisation and sector practice capability in working with participants with exceptionally complex support needs by leading and / or actively contributing to knowledge exchange activities.
• Develop and expand stakeholder relationships in the community to remove barriers and improve community access for participants with exceptionally complex support needs.
• Build capabilities through the application of knowledge and skills to enhance service policy, procedures and knowledge and skills of those who provide direct support to participants with exceptionally complex support needs.
• Provide advice which builds the skills, capability, confidence and responsiveness of support coordinators to support a participant with exceptionally complex support needs.
• Undertake timely adjustment and provision of support coordination directly to NDIS participant with exceptionally complex needs during points of crisis (including rotating rostered out of hours support).

About You:

You have the passion, dedication and drive to make a difference in achieving high quality care with demonstrated success in working with clients. To be suitable for this position, you will possess:

• A University degree qualification in a relevant field e.g., Social Work, Occupational Therapy, Nursing, Psychology
• Extensive experience working with people with complex needs; including mental illness, homelessness, disability, substance abuse, ABI or clients with current or previous involvement in the forensic system.
• A comprehensive understanding of the key issues affecting people with multiple and complex needs.
• Demonstrated ability to deliver training and/or a willingness to develop the skills to deliver training.
• Demonstrated ability to engage, liaise, negotiate and work collaboratively with a range of internal and external services and stakeholders - displaying a high level of interpersonal skills, negotiation and conflict resolution skills.
• Excellent writing and presentation skills, proven administrative and organisational skills and the ability to manage competing priorities within specified and tight timelines.
• Experience in, or willingness to develop skills in specialist support coordination for people with complex needs.
• A current driver's license.
