Vida Connects is a growing disability support provider based in Sydney, and we're looking for an organised, people-first Support Coordinator to join our team on a full-time or part-time basis.

You'll be the go-to person for participants navigating the NDIS - helping them understand their plans, connect with the right services, and get the most out of their funding. It's a role that sits right at the heart of what we do, and no two participants are the same - which means your days won't be either.

What you'll be doing

You'll work closely with participants and their families to coordinate supports that actually make sense for their lives. That means understanding their goals, linking them with providers, managing service agreements, and keeping things on track. You'll also be a key contact point between participants, support workers, and external services - making sure nothing falls through the cracks.

Day to day, you might be:

  • Helping a participant understand their NDIS plan and what's available to them
  • Connecting people with services and providers that match their needs and goals
  • Setting up and managing service agreements and service bookings
  • Monitoring plan budgets to make sure funding is being used effectively
  • Attending planning meetings, reviews, and team discussions
  • Troubleshooting issues, following up on gaps, and advocating for participants when needed
  • Building strong relationships with participants, families, and external providers

We're looking for someone who:

  • Genuinely cares about people and treats everyone with dignity and respect
  • Is organised, detail-oriented, and can juggle multiple participants without dropping the ball
  • Communicates clearly and confidently - whether it's with a participant, a family member, or a provider
  • Understands the NDIS framework and how plans, funding, and support categories work
  • Can work independently but also knows when to ask for help or loop the team in
  • Brings a positive attitude and a willingness to learn

What you'll need

  • Certificate IV in Disability or Individual Support, or relevant experience.
  • Experience in support coordination, case management, or a similar role within the disability or community services sector
  • A strong understanding of the NDIS, including plan management, service bookings, and the NDIS Practice Standards
  • A current NDIS Worker Screening Check (or willingness to obtain) and Working With Children Check (or willingness to obtain)
  • Strong computer skills, including experience with CRM or client management systems
