The Case Support Worker provides culturally respectful support to clients and carers, assisting with transportation, appointments, and participation in activities aligned with the client’s Case Plan and Targeted Care Package (TCP) funding. The TCP program aims to help children exit or stay out of residential care. Working closely with the Key Worker and Team Leader, the Case Support Worker helps achieve identified goals within the allocated support hours.
The Case Support Worker assists Key Workers and Team Leaders in delivering culturally respectful services under the Targeted Care Package (TCP), including transportation, appointments, and activities aligned with client case plans. They provide additional client and carer support, maintain accurate case notes, and ensure compliance with TCP goals, VACCA policies, and legislative requirements. Actively participating in team meetings, supervision, and professional development, they uphold VACCA’s values and commitment to Aboriginal self-determination. They build strong relationships with clients, carers, and stakeholders, fostering a culturally safe environment and supporting TCP objectives.
