Streetlink is a free and confidential youth health service that provides a safe place for young people aged 10 to 25, who are homeless or at risk of becoming homeless, to seek information and support, connection to services, or just chat with someone who cares. Our medical clinic, and drug and alcohol counselling service work in a collaborative and holistic manner to support young people across metropolitan Adelaide. 

The Community Nurse will be providing high quality, evidenced based care at an advanced nursing level as part of a multidisciplinary team. You will be providing primary health and specialised Alcohol and Other Drug treatment services for vulnerable young people and their accompanying children.

This is a Permanent Part Time Position 3 days per week.

Hours of work will be clinic based.

Responsibilities and duties will include:

  • Comprehensive clinical nursing services including support for AOD and mental health issues, immunisations, ante and post-natal care, child and adolescent health issues, sexual health and contraception, health screening and health education.
  • Referrals to other health programs, counselling and advocacy services.
  • Accurate and ongoing client medical records utilising Medical Director and other software.
  • Work within a multidisciplinary team to provide an integrated treatment model for homeless, vulnerable and at risk young people (and their children) with a focus on primary healthcare and alcohol and other drug (AOD) treatment.
  • Provide clinical leadership within the program and in partnership with relevant key clinical stakeholders to provide quality, coordinated and timely patient care and outcomes.

About You

You will bring the following to this position:

  • Current AHPRA Registration as a Registered Nurse
  • Strong understanding and knowledge of clinical care services to vulnerable population groups, with a focus on young people (12-25) and children
  • Experience in community service settings, working with vulnerable population groups
  • Ability to rapidly and continuously evaluate client’s condition and response to therapy and modify the management plan when necessary.
  • Demonstrated clinical experience and knowledge
  • High level understanding and knowledge of Australian standards for practice

Experience in a community service setting is desirable.
