In this role, you will work independently and as part of a multidisciplinary team to support consumers with low to moderate mental health needs, helping them to avoid re-admission or minimise the potential for hospital admission. You will engage with consumers through the structured 12-week program, using a goal-based recovery model. The program is adaptable, with a consumer-led pace. Your responsibilities will include providing generalised and short-term supports to help consumers build resilience and stay engaged with their recovery.
We are looking for a people-focused, adaptable, and self-motivated individual who can thrive in an off-site, independent role. You will have strong knowledge of services available on the Central Coast and be comfortable linking consumers with these resources. Success in this role requires:
