About the Role

This position is responsible for case management and the delivery of person-centred home and community care services. You will facilitate the delivery of person-centred, quality services to support our clients to live independently in their homes for longer while staying connected to their local community.

Key Accountabilities

  • Advocate for client choice
  • Plan services for clients that meet their individual needs within their available funding
  • Identify and escalate vulnerable and at-risk clients, including those with complex care and planning needs
  • Participate in local governance, continuous improvement, and risk management activities

Your Qualifications and Experience

  • Certificate IV qualification in a relevant field (qualifications in aged care or health care preferred); equivalent experience and relevant skills will also be considered
  • Current First Aid Certificate
  • Current and unrestricted Australian Driver’s Licence
