This is more than just a case management role. You’ll be the trusted point of contact, coordinating personalised care and building meaningful relationships that make a real difference every day.
Uniting Home & Community Care supports older people to remain living at home, connected to what matters most. We take the time to understand each person’s story, working alongside them to deliver tailored support that promotes independence, dignity and a sense of belonging in their community.
You’ll be the central point of contact for your clients, supporting them to access the right services and get the most out of their funding, while staying connected to their community.
