Homelessness Melbourne Metro is committed to providing person centred care, trauma informed practice and a strengths-based approach to improve the health, well-being and housing outcomes for people experiencing homelessness.
The Homelessness Melbourne Metro region provides a comprehensive range of residential & community services in the inner city of Melbourne across six sites in St Kilda, Footscray, North Melbourne, West Melbourne and Kensington
Community programs include the Crisis Contact Centre, Community Outreach Services for single adults, support for older people in public housing, rough sleeper & street sleeping initiatives, community participation and AOD support.
Residential services operate from Foley House in Footscray, the Open Door in North Melbourne and Flagstaff in West Melbourne.
Homelessness Melbourne Metro is supported by co-located services including the TSA Chaplaincy program, Bolton Clarke primary health service and NDIS support coordination.
The Residential Case Management Services Team Leader is part of a collaborative team, working in partnership to provide support to single men experiencing primary homelessness and rough sleeping.
With direction, exercise case management service delivery responsibility and ensure the mission and values of The Salvation Army are reflected throughout.
Work in collaboration with the Residential Community Support Team in establishing a culture of service delivery that is Person Centred, Strengths Based and Trauma Informed.
