The Bridge Programme's residential rehabilitation service operates over two sites:

Assessment: Approximately three weeks, based at Bridge House in Highgate.

Harry Hunter Rehabilitation Centre: A structured thirteen-week programme, based in Gosnells

Extended: For those seeking more time in a residential setting, based in Gosnells.

Non-residential options: The Bridge Programme offers two services on a non-residential basis:

ABOUT THE ROLE

The Social Practitioner based at Harry Hunter Rehabilitation Centre will provide case management, AOD counselling, treatment- and exit-planning support to individuals experiencing problematic substance use in order to reduce harm and facilitate the cessation of use of substances of addiction. Individualised treatment plans are to be developed in collaboration with service consumers.

HOURS AND SALARY

This is a permanent full time position, 38 hours per week.

Salary and conditions of employment are in accordance with Social, Community, Home Care and Disability Services Industry Award 2010, Level 5

The successful applicant will possess the following skills and attributes:

  • Tertiary qualification in the health/social sciences and/or extensive clinical practice.
  • Sound knowledge of the alcohol and other drug sector.
  • Sound knowledge and understanding of Aboriginal culture and people.
  • Demonstrated high level assessment and counselling skills.
  • Demonstrated high level case management skills.
  • Well developed interpersonal, verbal and written communication skills.
  • Ability to work independently and collaboratively within a diverse team.
  • Experience in facilitating education groups.
  • Practical knowledge of available allied community sector services for the AOD client group.
  • Demonstrated experience working with clients with complex needs including co-occurring AOD and mental health issues.
  • Possession of a current driver's license and reliable vehicle.
