The role you could play

We are seeking a dedicated Complex Care Coordinator! You will provide Complex Care Coordination including AOD and mental health care coordination to Greenlight Plus clients. Additionally, you will collaborate with TSA and SHM Greenlight Plus teams as well as Access Health to achieve positive client outcomes.

Duties

  • Ensure clients receive services in practical and non-judgmental way
  • Establish effective relationships with clients that encourage participation
  • Provide support to clients consistent with program care coordination models
  • Develop individual care plans with clients, health and other professionals that include a range of interventions
  • Provide clients with information, education, advice practical support and assistance
  • Assist clients to access health services such as physical/mental health, drug and alcohol, counselling and make use of eHealth services

​This is a permanent part-time position, 22.5 hours per week, based in St Kilda, VIC. Salary and conditions are in accordance with SCHADS Level 5 Award.

The ideal candidate will have:

  • Degree qualification in Nursing, Social Work or related field is essential
  • Experience working with people with complex needs required
  • Understanding of and commitment to the mission and values of The Salvation Army
  • A current and valid Working with Children Check (required)
  • A current Victoria Driver's Licence
