The Salvation Army Youth Services offer an integrated suite of programs engaging with young people across Australia. Our services create intentional avenues for young people to explore opportunities, build support networks, and access, participate and contribute to their communities. Youth services provide a diverse range of programs which include housing and homelessness, driver training, education, employment and training, AOD support, social and community activities, specialist therapeutic responses and youth justice programs.

About the role

We currently have exciting opportunities for values-driven Case Managers to deliver high quality case management support services across Bass Coast and South Gippsland to young people including families with accompanying children who are at risk of, or experiencing, homelessness. This role provides integrated pathways of support for young people's physical, emotional, social, spiritual and psychological development.

You will successfully

  • Ongoing assessment is made of the client's issues, needs and circumstances from initial intake to exit
  • Individual case plans are developed in collaboration with clients
  • Clients are assisted to access support services and establish community supports, and where needed are assisted through advocacy with service providers
  • Progress and outcomes of individual case plans are regularly monitored and reviewed
  • Support an integrated team approach and assist as required with programs such as the Schools Beyond Bank therapeutic life-skills project and the CoSS Model project
  • Work to the direction of the Client Services Coordinator and the State Practice Lead on the development, implementation and review of therapeutic case plans
  • Ensure all practice is consistent with the trauma and Psychologically Informed Environment framework.
  • Participate in a least one portfolio area team and one strategic working group as required
  • Facilitate house meetings, meal nights, life & living skills sessions with young people as required

This is a permanent position. We are flexible in the work time type for this position. Therefore, we have available full time and part time (0.8 or 0.9 EFT) positions. These positions will be based at our Youth Services in Leongatha.

Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Award, Level 5.

You will have

  • A degree qualification in social work, social sciences or related field is preferred
  • Experience and competency in delivering case management practice within a homeless service setting
  • Well-written verbal and IT communication skills
  • Excellent organisation and administrative skills with capacity to balance workload, determine priorities and meet deadlines while also managing the needs of client crisis assistance requirements
  • A current VIC Drivers Licence
  • Valid VIC Employee Working with Children Check
  • Ability to provide proof of Eligibility to Work in Australia
