Thorne Harbour Health (THH) is an LGBTIQ+ community-controlled health organisation, governed by our members, and working for the communities we serve. Combining passion, commitment, and activism, THH aims to improve the health, social and emotional well-being of LGBTIQ+ communities and those living with, or affected by, HIV. THH is Australia’s oldest LGBTIQ+ and HIV organisation, having celebrated its 41st anniversary in 2024. THH operates in Victoria and South Australia.

ABOUT THE ROLE

This exciting role will provide culturally safe and affirming psycho-social support and case management to LGBTIQ+ community members with mental health conditions, associated functional disabilities, complex needs, and comorbidities, living in metropolitan Adelaide.

As a Community Mental Health Practitioner, you will possess considerable skills and experience in psycho-social support, case management, and recovery-based assessments and care planning, underpinned by person-centred, trauma-informed, strengths-based and family inclusive practices. You will also demonstrate an interest in, and commitment to improving the health and wellbeing of all LGBTIQ+ communities and individuals.
