About the Role

We are seeking a passionate, project-driven clinician to support the roll out of the My Care Partners project in South Western Sydney.

My Care Partners is unique to South Western Sydney and supports those living in our region with complex and chronic conditions who are at risk of poorer health outcomes and frequent hospitalisations by enabling a healthcare neighbourhood approach.

As the Integrated Care Enabler, you will work within a small, but dedicated team on the implementation of this innovative local program within general practice and in collaboration with South Western Sydney Local Health District.

You will provide a key role in liaising with members of the patient’s care team, connecting providers, ensuring effective communication and working closely with general practices to ensure each patient gets the care that they need.

Your strong interpersonal and stakeholder engagement skills will be vital for building and maintaining positive stakeholder relations with project partners, internal stakeholders, and with a variety of general practices as they are recruited into the program.

Note: A clinical qualification is a requirement for this role.
